- Facility name, address, and proposed opening date
- Age group and capacity plans, and session types offered
- Information for up to four applicants and a primary contact
- Residence-in-care details (family or group homes only)
- Selected provincial orientation date
- Supporting documents (incorporation, floor plan, insurance)
